Overview
​Lizzy Morton Clarity Assessment is committed to respecting the privacy and protecting the data of clients. I protect the privacy of all who share personal data with me via email, post, text, phone or my website. I will only collect and use personal data in ways that are described in this policy. I have strict policies and procedures in place to protect your data when you send it to me, and when I process it to provide you with the services you have requested.

The data you provide will be used only for the efficient provision of agreed upon services and it will not be shared with anyone else without your prior authorisation. You have the right to see what data I hold on you and request that any data held is amended or deleted. Please contact me if you have any concerns or queries.

I am registered with the Information Commissioner's Office, the regulator of the Data Protection Act 1998 and abide by its requirements.

What Information is Collected

Personal details (names, contact details, date of birth, gender, occupations).

Educational history and relevant developmental and health information. In the course of providing dyslexia assessments, you may voluntarily provide us with specific health or educational information relevant to the assessment process. This information is only used to better understand your needs and offer personalized recommendations.

Results from assessment tests.

Before the assessment data is collected using a family and school questionnaire. These are completed via Google forms and stored on Google. Please see Google’s privacy policy for more information.
Why Information is Collected

To conduct an accurate learning assessment.

To prepare and deliver a written report.

To maintain necessary records for legal and professional obligations.
How Information is Stored

Digital records are stored securely with password protection and encryption where possible.

Paper record forms of assessments are kept in locked storage and are destroyed once the final report is provided.
How Long Information is Kept

Reports are digitally stored until:

Age 24 if assessed while under 18, or
-
Six years after assessment if aged 18 or over.

After this time, all records are securely destroyed.

You may request access to the personal information held about you/your child.

You may request corrections to inaccurate information.

You may request deletion of records, provided this does not conflict with legal obligations.

Sharing Information

Information will only be shared with third parties (e.g. schools, universities, other professionals) with your written consent, unless required by law or safeguarding concerns.
