TERMS & CONDITIONS

Assessment​

  • A non-refundable £200 deposit is required to secure an assessment booking. The remaining balance is due 5 days before the assessment.

  • If no diagnosis is made or a report is not required, the client may decline the report and will be partially refunded. In this case, the total fee will be £240 for the assessment only.

  • Clients must ensure that a standard sight test has taken place in the last 2 years. ​

  • Clients must ensure that the family and school questionnaires are received at least 48 hours before the assessment is due to take place. This background information forms a vital part of the assessment process and enables the assessor to select suitable assessment tools to use. Failure to provide these documents may result in the cancellation of the assessment. It may be necessary to cancel or reschedule the assessment if the aforementioned documents are not made available.

Reports & Diagnosis

  • It may be possible to provide some general feedback on the day of assessment but test results require careful interpretation before a diagnosis can be made. 

  • A detailed written report (approx 25 pages), confirming diagnosis or not, will be provided following the assessment, usually within 2 weeks.

  • It is the responsibility of the client/parent to keep a secure copy of the report for future use.

  • Please be aware that diagnosis is not guaranteed on booking an assessment. There is a strict set of criteria that is applied to making a diagnosis.

Confidentiality

  • All information shared is treated in strict confidence and used only for the purposes of assessment and reporting.

  • Information will not be shared with third parties without written consent, unless required by law or safeguarding concerns.

Record Keeping & Data Retention

  • Raw test data, score sheets, and working notes will be securely destroyed once the final report is sent.

  • Assessment reports will be stored securely:

    • Until the client reaches age 24 (if under 18 at assessment), or

    • For six years after assessment (if 18 or older at assessment).

  • After this period, records are deleted/destroyed and copies cannot be provided.

Cancellation Policy

  • Notice Period: At least 7 days’ notice is required for cancellations or rescheduling.

  • Late Cancellation (less than 7 days’ notice): A fee of 50% of the assessment cost may be charged.

  • Non-attendance / same-day cancellation: The full fee will be charged.

  • Illness/Emergencies: Exceptions may be made at the assessor’s discretion.

  • If the assessor must cancel (e.g. illness), every effort will be made to reschedule promptly or provide a refund if rescheduling is not possible.
